Third-party laboratory, imaging, procedure, and specialist fees remain separate and are billed directly by those providers.
Important Information
PHW Specialty Clinic provides consultation-based primary and specialty care. Blood draws, specimen collection, injections, vaccinations, imaging, and other in-clinic procedures are not performed at PHW. When these services are needed, they must be obtained through appropriate external providers and may involve additional fees. Membership benefits, physician access, communication arrangements, consultation availability, and service limitations are governed by the applicable membership agreement and practice policies. Third-party services and associated charges are separate from PHW membership fees. PHW does not replace every service offered by a traditional, full-service primary care practice. Depending on your needs, maintaining a separate primary care physician and accessing other providers may be appropriate. Emergency and urgent medical needs requiring immediate or hands-on care must be directed to the appropriate emergency or urgent care service.

- VIP Membership includes: unhurried physician consultations, direct access to Dr. Malik, personalized care planning, test ordering and interpretation, referrals and care coordination.
- VIP Membership does not include: in-clinic procedures or testing, such as blood draws or imaging. These are performed by our trusted partner providers and billed separately by them, including to your insurance when applicable.
