    How It Works

     

    Taking charge of your health is a significant step towards achieving optimal well-being. Potomac Health & Wellness (PHW) simplifies the process of becoming a new patient, enabling you to access our exceptional concierge care services. For new patients, all initial appointments are conducted in-person at our clinic, regardless of whether you reside locally or are visiting from out of town. Subsequent follow-up appointments offer the flexibility of either in-person clinic visits or convenient online virtual visits, accommodating your preferences and ensuring continued access to our high-quality care.

     

    1st: Complete Both New Patient Online Forms

     

    New Patient Registration Forms 

     

    Medical Symptom/Toxicity Questionnaire (MSQ).

     

    After we review your new patient information, we will contact you within 3 business days to schedule your initial clinic visit.

     

    2nd: Secure Your Initial Appointment

     

    Upon scheduling your initial clinic visit, we require you to pay your one-time, non-refundable $150 reservation deposit. Your deposit will be automatically credited towards your initial clinic visit fees.

     

    We will capture your preferred billing credit/debit card information for secure storage. This card will only be used to settle outstanding your PHW patient account balances or clinic visit fees.

     

    3rd: Submit Your Health Information

     

    Log on to your patient portal and upload your relevant medical information for us to review 5 business days before your initial clinic visit appointment.

     

    At your initial clinic visit check-in, please present your current government-issued photo identification card and active health insurance plan card(s).

    Clinic Policies

     

    We value our relationships with our patients and strive to provide a respectful and compassionate caring environment for our patients. We expect our patients (and their companions) to also treat our staff, providers, patients, and office property with the utmost courtesy and respect. We are committed to providing you with exceptional health care services. We most appreciate your understanding and cooperation in adhering to our clinic policies.

     

    Tardiness: Patients who are more than 10 minutes late for scheduled initial/follow-up appointments will be automatically rescheduled and charged a $150/$75 no-show charge per late initial/follow-up appointment.

     

    No-Shows: Patients who no-show for scheduled initial/follow-up appointments will be automatically charged a $150/$75 no-show fee per missed initial/follow-up appointment.

     

    Cancellations/Rescheduling: All patient appointment cancellations or rescheduling requests must be communicated to PHW a minimum of 2 business days before scheduled appointments to avoid the no-show fee.

     

    Dismissals: Patients who are repeatedly late (on more than three occasions), patients who are repeated no-shows (on more than two occasions), or patients who are consistently rude, disrespectful, and over-demanding will be dismissed from PHW altogether.

     

     

    Concierge Care W/O Concierge Fees

     

     

    Thank you for considering Potomac Health & Wellness (PHW) for your health and wellness needs. We eagerly anticipate the opportunity to meet you. PHW operates as a no-insurance, direct pay concierge medicine clinic that is entirely free to join without the typical binding concierge membership or mandatory treatment subscription recurring fees. Our transparent pay-as-you-go model means that our patients pay for only the high-quality concierge care they receive per appointment, with no hidden or surprise clinic fees. It's worth noting that many health plans offer out-of-network benefits that reimburse our patients for their PHW clinic visit fees. Upon request, we are more than happy to provide you with insurance-friendly coded claims and payment information that you can submit to your health plan for reimbursement.

     

    PHW Clinic Fee Structure

     

     

    At Potomac Health & Wellness (PHW), we are committed to providing exceptional concierge-quality healthcare without the hidden costs and surprises often associated with traditional health care. Our transparent fee structure ensures that you are fully informed about the costs of your care before and after your appointment. Our clinic fees are determined on a patient-by-patient and appointment-by-appointment basis, taking into account the following factors:

     

    Patient Complexity: The level of complexity of your health issues and medical conditions presented per appointment.

     

    Provider Face Time: The dedicated face time your provider dedicates to you per appointment.

     

    Provider Medical Review Time: The time your provider spends reviewing and assessing your health records, information, test results, etc., per appointment.

     

    Services Rendered: The clinical analysis, diagnosis, treatment, and consultation services provided per appointment.

     

    Please note that all third-party patient ancillary health services, including labs, pharmacies, imaging, etc., are solely the financial responsibility of the patient/guarantor. PHW clinic fees are subject to change annually.

     

    Specific 2023 Clinic Fee Details

     

    Initial Clinic Visit (In-Person Only): Up to 90 minutes provider face time + 60 minutes patient medical information review time, including your in-depth consultation and treatment, initial diagnostic laboratory testing orders, signature VIP patient experience with concierge benefits. Fee: $450-$550. Please allow up to 1.5 hours for your initial clinic visit appointment.

     

    Follow-up Clinic Visit (In-Person/Online): Up to 45 minutes provider face time + 30 minutes patient medical information review time, including your in-depth consultation and treatment, detailed laboratory test results and interpretations, signature VIP patient experience with concierge benefits. Fee: $250-$350. Please allow up to 45 minutes for your follow-up clinic visit appointment.

     

    Optional Concierge Care Services: Prescription refills, third-party medical records review, third-party patient forms, medical records copies, disability determinations, dietary counseling, patient advocacy or special request letters, etc., are provided upon request for an additional fee, which is solely the financial responsibility of the patient/guarantor.     Patient Discounts

     

    Patient Discounted Fullscript Supplements: Patients are eligible for an automatic 15% off discount for Fullscript medical-grade supplements. Please contact us for more information.

     

    Patient Discounted Clinic Fees: Qualified patients are eligible for a 10% off discount on clinic fees per appointment. Please contact us for more information. 

     

    Payment

     

    We only accept credit/debit cards, including FSA/ HSA debit cards, as forms of payment.
